Optimizing the role and value of the interdisciplinary team: Providing person-centered end of life care


The National End of Life Care Programme has created the guide, Optimizing the role and value of the interdisciplinary team: Providing person-centred end of life care to support those working within, or alongside an interdisciplinary team in palliative care.  An interdisciplinary team (otherwise known as an inter-professional or multidisciplinary team) is increasingly recognized as a way to integrate and coordinate person-centered care in palliative care.  This guide describes the value of interdisciplinary teams and how they can be properly utilized in this setting.
